Independent Living Skills Trainer

Unlimited Care Inc. is looking for an Independent Living Skills Trainer (ILST), who is responsible for facilitating independence, community participation, and quality of life for Nursing Home Transition and Diversion (NHTD) Waiver Program participants.

Essential Job Functions:

  • Assist participant and Service Coordinator in the development of individual service programs, behavioral strategies, and other service provisions.
  • Plan and work with participant and Service Coordinator to identify individual outcomes and/or goals.
  • Utilize the UAS assessment tool to establish baseline participant functioning annually and maintain all supporting documentation.
  • Implement the service plan by providing each participant with training in Independent Living Skills insuring effectiveness of adapting each plan to changing needs and priorities.

Job Requirements:

  • Master’s degree in social work or psychology; licensed/registered PT, RN, SLP, OT; certified special education teacher; certified rehabilitation counselor. A minimum of one (1) year of experience completing functional based assessments of ADLs and IADLs, developing comprehensive treatment plans and teaching individuals with disabilities and/or seniors to be more functionally independent.
    OR
    Bachelor’s degree with three (3) years of experience completing functional based assessments of ADLs and IADLs, developing comprehensive treatment plans and teaching individuals with disabilities and/or seniors to be more functionally independent.
    OR
    Associate’s degree with five (5) years of experience completing functional based assessments of ADLs and IADLs, developing comprehensive treatment plans and teaching individuals with disabilities and/or seniors to be more functionally independent.
  • Team coordination and communication skills.
  • Basic computer literacy.
  • A valid NYS driver's license is required
